2412 - HOMEBOUND INSTRUCTION
| A. | Whenever a principal finds that a student will be absent from school twenty (20) days or more for health reasons, s/he should advise the family to make a referral requesting homebound instruction (Form 2412 F1). This referral will be forwarded to the Special Education Coordinator. Evaluation and team determination of a health handicap will result in an IEP to provide homebound instruction. (Refer to Model Policies and Procedures for the Education of Children with Disabilities, IEP-LRE.) | ||
| B. | Teachers and/or guidance counselors should be contacted as soon as possible. | ||
| C. | All assignments are made through the office of the Special Education Coordinator. | ||
| D. | Accurate time sheets must be kept of all cases. The time sheet is to be signed by the parent and attached to the completed voucher. | ||
| E. | Time sheets and vouchers are to be submitted by the end of the pay period. | ||
| F. | All books and supplies will be furnished by the school in which the student is registered. | ||
| G. | Instruction will be given only on days that school is in session. Each child will receive a minimum of five (5) hours of instruction per week. | ||
| H. | The instructor shall forward grades to the principal of the school the student attends by the end of the marking period. | ||
| I. | Certified personnel giving bedside instruction shall be paid as approved annually by the Board. | ||
| J. | Instructors will receive the mileage reimbursement at the established rate both to and from school and residence of the student. | ||
| K. | If there are any questions pertaining to payment for bedside instruction or mileage, the Treasurer should be contacted. |
